Dear Rev. Fr. Odchigue:

Greetings of peace!

I am expressing my sincere apology for committing mistakes in encoding the grades of some
of my students in Accountancy Program, section A23 on the OPIS this first semester of the
academic school year 2019-2020 due to confusion and overlooking these errors. I know I
have failed to carry out my responsibilities properly as their instructor in Business Laws and
Regulations (BL 212) for it took me awhile to write you this letter which I could have done
earlier. This is due to being too preoccupied juggling my work in Sangguniang Panlalawigan
as ----------- I am completely aware that my negligence to resolve this problem earlier would
create adverse effects to my students and to the university.

Nevertheless, I am fervently hoping that you would help me with predicament of mine. I
would like to ask your good office to allow me to correct the errors I have made on the OPIS.
The changes I intended to make are of relevance to the below stated list of students:
