FORMAL ENTRY OF APPEARANCE

and MANIFESTATION

COMPLAINANT, thru the undersigned counsel, unto this Honorable

Office, most respectfully states THAT:

1. The undersigned counsel has just been engaged by the compliant to

serve as her counsel in this case and to appear in hearings related to this case in
her behalf.

2. The undersigned counsel hereby formally enters his appearance as

counsel for the complainant in this case and moves that all pleadings, motions,
orders and processes and decision of this Honorable Office be addressed
to the office and residence of the undersigned counsel in FAIRVIEW
HOMES, SABELLANO ST., BASAK, PARDO, CEBU CITY 6000.

3. My client, Eden J. Canonigo, received late afternoon of June 2, 2021

almost at 6:00 PM the Notice of Conciliation Conference from the PRC setting
the conference at 10:00 o’clock in the morning of the same day, June 2, 2021. It
was impossible to respond to said notice on time.

4. My client also manifests that she will not enter into a compromise with
the respondent.

5. Due to the pandemic and the distance of our residences, my client sent
her conformity and manifestation thru email dated June 3, 2021:

(a) Email from my client as Annex “A” engaging my services as her legal
counsel in the instant case showing her conformity to my formal entry
of appearance and manifestation that she will not enter into a
compromise with the respondent;
(b) Letter addressed to me signed by my client stating the above
conformity and manifestation as Annex “B”;

WHEREFORE, it is most respectfully prayed of this Honorable Court

to direct that all pleadings, motions, orders and processes and decision of
this Honorable Court be addressed to the office and residence of the
undersigned counsel. Other reliefs as just and fair are also prayed for.
