AFFIDAVIT OF LOSS

I, ROWEL YEE MATUGAS, Filipino, of legal age, with address in Digos City,
Davao del Sur, Philippines, after having been duly sworn to in accordance with law do
hereby depose and say:

1. Sometime in 2013 or 2014 I cannot be sure anymore as it was a long time

ago, the bag (in which I placed my University of Cebu Identification Card with
student number 09112095) was left in the transportation van I and my
company used to go home to Consolacion. The bag cannot be retrieved after
my utmost effort. The driver said, immediately after I and my company had
used the van, other groups used the van.

2. I execute this affidavit for the purpose of fulfilling the requirement of the
University of Cebu for the release of my school records including but not
limited to Transcript of Record and Record or Certificate of Good Moral
Character.

IN WITNESS WHEREOF, I have hereunto set our hand this ______ day of
