3. That in compliance with the provisions of the 2022 Revised Student Handbook and the applicable provisions of CMO No. 63 s. 2017, I
hereby declare that I am giving my full consent / permission for him/her to join such activities provided that the same are sanctioned by Cor
Jesu College and provided further that I am subsequently provided with the usual Parent’s Consent Form which will not anymore require a
notarization from a lawyer.

4. That I am executing this affidavit to attest to the veracity of the foregoing information, in compliance with the school regulations, and for
whatever legal purpose this may serve.
