STUDENT PERFORMANCE CONTRACT

School Year 2023-2024
TO WHOM IT MAY CONCERN:

Science High School, willingly and voluntarily enter into a contract with all my teachers represented by my class adviser.
The main purpose of this contract is for me to improve my personality and academic performance hence achieve at least
a satisfactory grade in all my subjects this school year _SY 2023-2024_.
As part of this contract, I commit myself to do the following:
1. Attend all my classes and school activities/programs.
2. Participate actively in class discussions, group works and other learning activities.
3. Develop a good learning habit/study habit and to master the required competencies or skills in the different
learning areas.
4. Make projects, homework, assignments, and other requirements to the best of my ability and submit them on or
before the set deadlines.
5. Improve my knowledge, skills, and character by participating actively in school and community activities.
6. Learn through reading (at least 2 books within the school year aside from the textbooks), using computer,
internet, and other means of learning.
7. Observe all school rules, barangay, and city ordinances, and be a law-abiding citizen.
8. Always demonstrate proper behavior anywhere.
9. Show respect to fellowmen especially authorities, elderly and persons with disability.
10. Be responsible and accountable in all my actions.
11. Share my knowledge and talents with other students.
12. Be a model student and bring HONOR to my family, school, and community.

AGREEMENT
As the parent /guardian of ___________________________________________ presently enrolled in Grade _________ I commit:
1. To check on the educational progress of my child/ward with school personnel once a month
2. To attend the quarterly Card Giving and PTA Assembly
3. To actively support my child/ward in his/her education by checking on assignments, lessons, and other school
requirements, and seek consultation with the teachers.
4. To volunteer my time and support school/DepEd initiatives at least twice within the school year.
5. To explain in writing why my child/ward was absent on school day/s and coordinate with my child's/ward's
teachers to determine what activities must be undertaken to help my child/ward cope with missed lessons.
6. To immediately attend to requests from teachers for conferences/ consultations which affect my child's/ward's
academic development and well-being.
7. To immediately update the school for any changes to our personal information indicated in the enrolment form
e.g. address, contact number, etc.

3. To actively involve my parent/guardians in my education by updating them on what I have learned in school.
4. To minimize my absences to avoid the consequences of missed school days.
5. To refrain from any acts of bullying and to honestly and immediately report such acts to my adviser/subject
teacher
6. To attend and actively participate in the flag ceremony to express gratitude to my nation and countrymen.
