MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:


This Agreement made and entered into this 12th day of October 2022 at Dilag,
Tabuk City Kalinga, by and between:
DEPARTMENT OF EDUCATION(DepEd), -DILAG INTEGRATED
SCHOOL with office address at Dilag, Tabuk City, represented herein by the School Head,
IMELDA L. HABAN, herein referred to as “DepEd-Dilag Integrated School:”
-and-
JENYROSE SENICA, ADDRESS OF PUROK 3, BULANAO, Tabuk City
represented herein by _________________________, referred to as “SISTER”.

WITNESSETH
WHEREAS, the Department of Education (DepEd) has applied blended
learning modalities to ensure learning continuity. This way, students can continue their
education in remote setups through online/offline platforms and printed modules during
modular leaning and face to face modality.
WHEREAS, the Department of Education (DepEd), in collaboration with its
partners and stakeholders, shall ensure that all schools are conducive to the education of
children. The best interest of the child shall be the paramount consideration in all decisions
and actions involving children, whether undertaken by public or private social welfare
institutions, courts of law, administrative authorities, and legislative bodies, consistent with
the principle of First Call for Children as enunciated in the CRC.
WHEREAS teachers and learning facilitators especially in learning centers are
their substitute parents and are expected to discharge their functions and duties with this in
mind.
WHEREAS DepEd believes that one way of achieving an enhanced learning
environment and improving the quality of basic education is to enter partnerships that are
able to demonstrate a contribution to improve learning, development and wellbeing outcomes
for learners.
NOW THEREFORE, for and in consideration of the foregoing premises and
the terms and conditions herein set forth, the parties hereby agree to undertake the following:
1. Advisers shall orient students about the rules and policies of the face-to-face learning.
2. Advisers shall provide signed Consent forms from the parents as applicable.
3. Subject teachers shall monitor and update learning progress of the learner
4. Communicate student’s progress to the learner/parents/guardians through text, call and
home-visitations and during card giving day.
5. Issue a final copy of grade to the student upon completion of the requirements within a
prescribed period.
6. The learner submits accomplished activities to the adviser and all other related
requirements, which will serve as a basis for the computation of the academic performance to
the agreed dates of submissions.
7. The learner will be joining the face-to-face learning modality scheduled by the school and
quarterly exam schedule.
8. The student’ failure in submitting and complying with the requirements would mean
FAILURE in the subject/grade level.
9. As the Regional Order No. 07, s. 2018 from May B. Eclar, Ph.D., CESO V, Regional
Director, DepEd CAR dated November 13, 2018, entitled “BASIC GUIDELINES IN THE
DEVELOPMENT OF LEARNERS’ DISCIPLINE MANUAL”. The guidelines in attendance
and punctuality as stated in the Paragraph 1.2, Section 1, Chapter III, Part IV of 2000 DECS
Service Manual shall be observed.
10. Regularity of attendance and punctuality are required in all classes. A student who has
been absent or has cut classes is required to present a letter of explanation from his/her
parents or guardians or to bring them to school for a short conference with the section adviser
or guidance counselor as the case may be.
11. A pupil/student who incurs absences of more than twenty percent of the prescribed
number of class or laboratory periods during the school year or term should be given a
failing grade and given no credit for the course or subject
12. Habitual tardiness especially during the first period in the morning and in the
afternoon shall not be allowed.

NOW THEREFORE, in consideration of the foregoing program, both parties shall strictly
follow the above terms and conditions:

IN WITNESSES WHEREOF, the parties hereto set and signed this document on this 12 th day
of October in Dilag Integrated School, Dilag, Tabuk City, Kalinga.
