AFFIDAVIT OF UNDERTAKING

I, HOMER A. BANA, of legal age, Filipino citizen, single with residence

and postal address at St. Alphonsus de Ligouri School, Inc. formerly Youngsville
Learning Center of Youngsville Subd. Igpit, Opol, Misamis Oriental, do hereby
states that:

1. I am the School President of St. Alphonsus de Ligouri School , Inc.,

an educational institution duly established with the laws of the Republic of the
Philippines with business/school address at Youngsville Subd. Igpit , Opol,
Misamis Oriental.

2. In compliance with the Limited Face-to-Face Classes of the

Department of Education, Implementation Plan made by the faculty which
outlines the steps the school should take when accomplishing a shared goal or
objective for the school year, 2022-2023 as a response to the Implementation of
the Limited Face-to-face Classes which will be adopted by the institution.

3. Accomplished School Safety Assessment Tool (SSAT Checklist) for

the Pilot Study on the Face-to-Face Learning Modality was prepared by the
faculty and was consulted with parents for their information and guidance
purposes.

4. In furtherance thereof, Contingency Plans were also prepared by the

faculty for reclosing and reopening of the school in case of COVID-19 resurgence
in the community.

5. This Affidavit of Undertaking is executed in good faith with all good

intention of compliance thereof and without intent of evasion and for purposes
of compliance with the Limited Face-to-Face Learning Modality of the
Department of Education as implemented by St. Alphonsus de Ligouri School,
