INFORMED CONSENT FORM

STUDY TITTLE: A Comparison of the Effectiveness of Technology- Based Instruction


and Lecture Demonstration in Dance Education

PURPOSE: You are invited to participate in a research study conducted by Yvette Eve
R. Villanueva, MAPEH teacher of Pardo Night High School. The purpose of this
research is to compare the effectiveness of Technology-Based Instruction with Lecture
Demonstration on Dance Education.

BENEFITS: Grade 10 students will be used as the respondents of the study, they will be
given rubrics to rate their performances. Scores will be credited as part of their
performance for the covered quarter. students may not get any monetary amount from
participating in the study but his/her participation will most likely to help find the answer
and maybe benefit the future generations.

COMPENSATION: there will be no reimbursement given for the activity will only be
done during their MAPEH time inside the school campus. But a certificate of recognition
and participation plus a grade for performance task will be given to the participants.

PROVISION FOR INJURY: there will be an available aid from the school clinic for any
untoward incidents. It will be given free to the students as a result of participating in the
study.

VOLUNTARINESS OF PARTICIPATION : When you are invited to participate a study,


you have the choice to be informed about the study so that you can decide whether you
want to participate or not. Feel free to ask the researcher if there are words you do not
understand. Your participation is voluntary. You can opt not to continue or not to join.
You are not forced to do so. You can also access your records or data ant time you
wish to.
CONFIDENTIALITY: Information and data gathered in this study is 100% confidential
so as to the personal information of the respondents. The data collected will be saved in
an external drive and stored in a safe place by the researcher. Information contained in
your records may not be given to anyone unaffiliated with the study in any form that
could identify you without your consent. You can access your data and records of the
study anytime.

Ethical Consideration

The following ethical principles were observed by the researcher: (1) Respect, (2)

confidentiality. (3) non-harming, and (4) honesty.

First is respect, Respect for person. It is a positive feeling or action shown towards

someone or something considered as important. The researcher values the diversity of

the respondents and maintained a high respect for them. Next is confidentiality. In seeking

information, it is researcher’s task to keep the data from the respondents confidential

specially if the respondents want to keep his/her identity unknown. It should not be shared

or given to anyone without the permission of the respondents. The third one is that the

researcher is non-harming. Upon the process of gathering data the researcher sees to it

that it maintains a harmonious relationship with the respondents. And the last in the list is

honesty. The researcher was honest enough to put up the exact words and data from the

respondent’s answers during the performance task and the interview.

Additional points to consider are respondents should participate on the basis of

informed consent. It involves providing sufficient information and assurance about taking

part of the study and allows individual to understand the implications and to be fully

informed, considered and freely given decision about the study. More so, students may

not get any monetary amount from participating in the study but his/her participation will

most likely to help find the answer and maybe benefit the future generations. The

participants will have the right to access any time his/her data and records of the study.
