INDIVIDUAL CONSENT LETTER

Infosys Limited having its registered office at Electronics City, Hosur Road, Bangalore – 560100, including its
subsidiaries, associates and affiliated companies (collectively referred to as “Infosys”) has a requirement to collect
Personal Information("PI") and Sensitive Personal Data or Information (“SPDI”) from you (the “you/your”) in
accordance with the Information Technology (Reasonable security practices and procedures and sensitive personal
data or Information) Rules,2011 (“Rules”).

Type of PI/SPDI collected

Infosys would be required to collect the following PI/SPDI from you for the purposes as specified below:

• Biometric details – Fingerprints

• Photograph

Purpose

Infosys may hereinafter be required to process, disclose or transfer the PI/SPDI for the following purpose :

• Infosys would require your biometric details – Fingerprints, in order to grant you physical access to the Infosys
premises.
• The biometric data will be embedded in your smart card. The smart card will enable you to have physical access
into the Infosys premises and will record your attendance.
• Infosys would require your photograph to establish identity.

Data Recipients

The biometric data will reside only on the smart card, and the card will be in your possession.

Photographs will reside in Infosys servers hosted in India and may be accessible to authorized internal recipients in
Infosys.

Security Measures

Infosys adopts reasonable security practices and procedures, in line with the international standard ISO 27001 in
order to protect your PI/SPDI.

You recognize and understand that there is no compulsion to provide Infosys with PI/SPDI and any and all
PI/SPDI provided by you would be provided on your own volition and as per the requirements to provide such
PI/SPDI.

In pursuance of the foregoing, you hereby acknowledge and understand that the PI/SPDImay be processed in the
above-mentioned manner and hereby consent to such processing of the PI/SPDI.
