AFFIDAVIT OF SERVICE
(Under Order V rule 15 (1)
I, JOHN KOMBE a resident of Mombasa and of P.O.BOX 54-80109 Mombasa in the Republic
of Kenya do hereby solemnly make oath and state as follows:
1. THAT I am a Court Process server with the firm of Omollo Onyango & Company
Advocates situated at Danicha Plaza, 3rd floor, Mombasa Malindi highway at Mtwapa,
duly authorized to serve court processes and therefore competed to swear this affidavit.

2. THAT I received from the said law firm a copy of an Application on 27th May, 2019 with
instruction to effect service upon the 2nd and 3rd Defendant’s/Respondent’s office herein.

3. THAT on the same day I proceeded to the 3rd Defendant's office in Kilifi Town, next to
the Governor's office where I met the secretary who declined service and insisted that I
serve the application to the 2nd Defendant.

4. That on the same day I proceeded to the 2nd Defendant's office at NSSF building 9th
floor where I met the secretary and after introduction and explaining the purpose of my
visit, she accepted service of the said documents by signing at the front of my copy which
I retuned duly signed.

5. THAT satisfied that I had done proper service, I now return to this Honourable Court the
above document as duly served.

6. THAT what is stated herein is true to the best of my knowledge, information and belief.
