CONTRACT AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This CONTRACT AGREEMENT is made and entered into by and between:
GRT TRUCKING & HAULING SERVICES, herein represented by JUDITH B. GO, of legal
age, married, Filipino and a resident of 1304 Datoc Compd., Digos City, Philippines, hereinafter reffered
to as the FIRST PARTY.

likewise of legal age, married, Filipino and a resident of ____________________________
_______________hereinafter referred to as the SECOND PARTY.

WITNESSETH:
WHEREAS, the FIRST PARTY has a contract projects with the DPWH that needs hauler for
sand and gravel aggregates;

WHEREAS, the SECOND PARTY owns hauling Dump Trucks whom he offers and willing to
haul the said aggregates for the FIRST PARTY’s project, and the FIRST PARTY accepts the offer;

WHEREAS, the view of the foregoing premises, the parties have hereunto agreed to enter into a
Contract Agreement subject to the following terms and conditions:

1. The SECOND PARTY shall haul sand and gravel aggregates from Dulag-Remedios quarry of
the FIRST PARTY to be delivered to the project site within Barangay Dulag, butuan City;
2. The SECOND PARTY will provide his own dump truck with a load volume of 14 cu.m. to 16
cu.m. for every truck load at a price of ₱250.00 cu.m. @ 35 km.
3. The SECOND PARTY shall submit Statement of Account/Billing with the attached duplicate
copy of Delivery Receipts (DR) to the FIRST PARTY’s office every Saturday and payment will
be every Friday.
