AFFIDAVIT of UNDERTAKING
I, MARJORIE SILVA RUBIO, Filipino, of legal age, single, and with residence and
postal address at 1300 c m Recto Ave. Santa Cruz, Manila, Philippines after having
been duly sworn to in accordance with law , hereby depose and state :

1. That I am the duly authorized representative of KAREN CECILIA C. TAN, the

latter being the owner of the residential structure located at KS1 Faith
community Center IIB, Alapan Road, Barangay Navarro, City of General
Trias, Cavite.

2. That, the said owner did not anymore engage the services of any contractor or
a construction firm in relation to the construction/improvements of the above-
mentioned residential structure due to the structure has already constructed;

3. Thus in view of the foregoing premises, the said owner could not present a
Contractor’s Affidavit / Construction Agreement in relation to the application
for the Occupancy Permit with the Office of the Building Official / Engineering
Office of General Trias Cavite;

4. That I hereby assure and guarantee under oath, and for and in behalf of the
owner KAREN CECILIA C. TAN that she will not hold the Engineering Office
of General Trias or Office of the Building Official liable in case of any
untoward or unexpected incident that might take place in the future, in
connection with the construction/improvements of the above-mentioned
residential structure notwithstanding the issuance of the Occupancy Permit in
my favor in the absence of such Contractor’s affidavit / Construction
Agreement;

5. That I am executing this affidavit to attest to the truth of the foregoing

declaration and for such other legal intents and purposes this may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 26th day of

September, 2019 at the City of General Trias, Cavite, Philippines.
