Re: Modification RA KM of HRD Entrance
Door (Drop Box Installment)

Purpose:
To get obtain BOD’s approval regarding modification of HR entrance door by install the drop box in
the backward of the door with total cost amounting to IDR 1.210.000, -.

Background:
Recently, HR room is a restricted area and since the door is closed, we need to having access to enter
the room, sometimes the employee need to put down or returning the document related with HR
matters with entering the room. By having modification of HR entrance door by install the drop box
in the backward of the door we make a limitation access to enter the HR room and make easy and
smooth when returning of HR documents. The drawing plan is as attached.

Analysis & Budget:

Based on our drawing and BQ calculation the works of modification of HR entrance door by install
the drop box in the backward of the door only can be done by a contractor, and the price of this
works is not expensive due to the scope of the works is not too large.

The quotation from vendor as follow (price include VAT 10%):

sandblast sticker at HR room, overall their work quality and time accuracy is very good.
For Budget allocation, we will use from Repair& Maintenance Building explained as follows:
