That our son/daughter will participate in the coming 1st Provincial-wide DRR-CCA Children’s Festival on
December 5-7, 2017 at Villa Maria Luisa Hotel, Dawis ,Tandag City.

That during the said date and activity, the coach has the responsibility in implementing the safety and
security of the students.

It is stressed, however, that whatever circumstances that is out of grasp and unintentional, we will not
held the coach liable.

As a proof of trust, we will sign this waiver.
