GUIDELINES IN THE ADMINISTRATION OF THE DIVISION UNIFIED QUARTERLY TESTS

Rationale:
Quarterly tests are provided as guage to determine the progress of students’ mastery level in
every subject area. These tests are used to evaluate student learning, skill level growth and academic
achivements at the end of the quarter.

The results of the quarterly tests are important since they provide picture of the strengths and
weaknesses of the students in every learning area, which could then be the bases for learning
intervention activities. The authenticity and validity of these test results largely depend on the quality of
the test materials used, and how these tests are conducted in the classroom.

As such, Surigao del Sur Division sees the need to come up with a division unified quarterly
test and its standardized administration across all schools in order to achieve objective and meaningful
results.

Objectives:

- to establish uniformity in the administration procedures in the conduct of the quarterly test;
- to provide objective measures to assess students’ academic achievement in every learning
area;
- to provide comparability of students’ academic achievement as bases for benchmarking across
schools and districts; and
- to provide accountability to schools for their academic performance

Guidelines:
A. Distribution of Test Materials

1. The Division Testing Coordinator and Assistant DTC facilitate the distribution of CD
containing the electronic copy of test materials at the division office.

2. The PSDS/DIC and the District Testing Coordinator receive the electronic copy of
test materials at the division office, and keep custody and confidentiality of these
materials upon receipt until the reproduction and distribution of these materials to the
schools.

3. The PSDS/DIC and District Testing Coordinator with the assistance of the School
Testing Coordinators and all ADAS facilitate the reproduction of the test materials at
the district office.
4. Every school shall provide the district office with the materials needed for the printing
of the test materials like bond paper and ink depending on the volume of the test
materials to be reproduced for a particular school. Schools may also lend their printer
or any printing machine in case the district office needs more.

5. The reproduction of Test Materials are in two sets: Set A and B. (Please see the
attached Board Work.)

6. The PSDS/DIC and DTC facilitate the distribution of the hard copy of the test materials
to the School Heads together with their School Testing Coordinator at the district office
early morning on the day of the examination except for those from far-flung schools,
which is done ahead in the afternoon before the examination day.

7. The School Head and STC receive and keep custody of the test materials upon receipt
at the district office until distribution to the Room Examiners.

8. The School Head and STC facilitate the distribution of test materials to the assigned
Room Examiners first thing on the examination day as reflected in the schedule.

9. The assigned Room Examiners are coming from the nearest school regardless it is
secondary or elementary. In case the number of teachers of a particular school does
not suffice the need of the neighbouring school, the PSDS/DIC and DTC may assign
teachers from other schools within the district.

10. The Room Examiners receive and account the test materials from the School Head,
and keep custody of these materials for the whole duration of the test.

11. One Division Personnel bearing the copy of the ANSWERS KEY is deployed in every
district to facilitate the distribution of the said materials to the School Testing
Supervisors at the district office first thing in the morning on the examination day.

12. The assigned School Testing Supervisor (STS) is coming from the nearest school
regardless it is elementary or secondary. One STS is deployed in every school except
for integrated schools and big secondary schools which have two (2): one for the
elementary and one for secondary for IS, and one for JHS and the other one for SHS
for big secondary schools.

13. The School Testing Supervisor receives the copy of the answer key from the assigned
DO personnel, and facilitates the distribution of the said materials to the Room
Examiners during checking of answer sheets.

B. Test Proper

1. All schools are strictly required to adhere with the examination schedule provided by
the Division Office. In no case that schools change or modify the prescribed schedule.
2. Seats are arranged in six (6) or eight (8) columns depending on the class size, and are
spaced enough with the last row close to the walls.

3. Test materials are distributed in an interval basis as Set A and Set B.

4. There are two board works, one for Day 1 and the other for day 2. The board work for
the day is posted on the board before the examination starts.

5. The examination starts and ends in all levels on time as reflected in the schedule.

6. Examinees answer the test simultaneously within the time allotment as provided in the
examination schedule.

7. The Room Examiner collects the Answer Sheets after the time allotment for every
subject ends. In the event an examinee has finished earlier, he/she has to review
his/her paper until the Room Examiner gives a go signal to proceed with the next part.

8. Cheating in any form is strictly prohibited. An examinee caught cheating after the third
warning shall be ushered to the School Head’s Office for some disciplinary measures
after the test session ends.

9. Checking follows right after the tests end as indicated in the examination schedule, and
examinees are dismissed all together after.

C. Checking of Answer Sheets and Reporting of MPS

1. The School Testing Supervisor with the help of the School Head and School Testing
Coordinator distributes the copy of the answers key to the Room Examiners once the
test starts. Only the copy of the answers key for the subjects tested for that day are
given to the Room Examiner.

2. The Room Examiner facilitates the checking of answer sheets together with the
examinees as indicated in the examination schedule, and consolidates and records the
scores in the Quarterly Test MPS Report Form right after.

3. The School Testing Supervisor collects the accomplished Quarterly Test MPS Report
By Grade Form after every test session in the morning and in the afternoon, and
accomplishes the School Quarterly Test MPS Report Form together with the School
Head and School Testing Coordinator.

4. After having all consolidated the MPS in all subjects, the School Testing Supervisor
submits the accomplished School Consolidated Quarterly Test MPS Report Form to
the PSDS/DIC and District Testing Coordinator, and furnishes copy of the said report
to the School Head.
5. Submission of the School Consolidated Quarterly MPS Report shall be made ONLY on
the second day as reflected in the examination schedule, and first thing on the
following day for schools without internet connectivity.

6. The PSDS/DIC and District Testing Coordinator consolidate all the quarterly test
School MPS reports, and submit the accomplished District Consolidated Quarterly Test
MPS Report Form to the Division Testing Coordinator ONLY on the third day from the
start of the examination schedule.

D. Monitoring & Evaluation

1. The School Testing Supervisor together with the School Head and School Testing
Coordinator monitor the overall conduct of the quarterly test in the school level, and
accomplish and submit to the PSDS/DIC and District Testing Coordinator the
Monitoring and Evaluation Tool provided.

2. The Division Personnel deployed in every district together with the PSDS/DIC and DTC
monitor the overall conduct of the quarterly test in the whole district, and consolidate
and submit to the Division Testing Coordinator the M&E Tool Provided.

3. The Division Testing Coordinator consolidates the District M&E Reports, and submits
the said document to the Chief Supervisor of the Curriculum Implementation Division.
