To whom it may concern,

I, the undersigned name is sending you this letter as notification that I am authorizing the bearer
of this letter, Ms. RIZZA JOY V. DIZON, my sister to submit the original copy of my
authenticated PRC License in your office since I won’t be able to submit it personally because of
an important reason.

I have no objection for Ms. Dizon to sign any papers that has connection with the submission of
the said document.

I have enclosed my Unified Multi-Purpose ID and TIN ID copies for your further reference.
