husband abroad this vacation. He is working in US and cannot come here in the
Philippines, so we decided that I’ll be the one to go there. I will have my vacation leave
from April 15 – May 31, 2019 inclusively.

I am LEYVERLYN L. CALOSA Teacher I of San Pioquinto Elementary School,

Malvar District.

My husband who is working abroad will be going home by August. As a couple,

we are praying to have children of our own. As per advise of our OB - Gyne, when my
husband took his vacation in the country we must undergo in a rigid medical
examination. Furthermore, she advise me to file a leave of absence from work to avoid
stress.

In this regard, may I request from your good office to allow me to file leave of
absence for (2) two months to give way for the medical procedure as advised by my
physician.
