I, ROMEL AGRES BERDIDA, of legal age, married, Filipino citizen, and a
resident of Bgy. Poblacion, Bataraza, Palawan after having sworn to in accordance
with law, hereby depose and state:

1. That I am the registered owner of a business under the trade name

RIVERSIDE INN located at MARANGAS, BATARAZA, PALAWAN;

2. That the above-said business was duly registered at the Department of

Trade and Industry (DTI) on 12 February 2016 and was issued Certificate
of Registration with No. 03950516;

3. That at present I am causing the cancellation of the said business due to

the fact that the business has been non-operational since _______ (please
insert date) and in support thereto I am executing the foregoing affidavit;

4. That I am executing this affidavit to attest the truthfulness of the foregoing

declarations under penalties of perjury to respectfully request the DTI to
CANCEL Certificate of Registration, No.03950516 and for other legal
purposes it may serve.
