Task/operation and location (including any tools/equipment in use):
The main operation is to set up and pre-cast manholes prior to storage/installation.
Small hand tools and manual handling will be used to erect/set the formwork and rebar.
Concrete transit mixer trucks will be used to deliver, directly by chute, the concrete and a
vibrating poker and tamping will be used after pouring to ensure there are no cavities.
Crane c/w lifting gear and flatbed truck will be used to move the manholes into position.

Controls:
A method statement must be developed for the job.
Check false work and formwork before pouring to ensure it is suitable and safe.
Do not allow concrete to be poured too fast.
Minimize number of people in working areas especially during pouring.
Manual handling techniques must be taught to the workers.
Wherever possible, minimize human involvement by using mechanical handling methods
Worker in contact with formwork/rebar must wear hardwearing leather gloves.
Workers must be informed of dangers when items are being lifted in their vicinity.
Reversing alarms must be fitted to all plant machinery.
Vehicles must have a banks man when reversing and/or near other working groups.
During pouring all workers coming in to contact with concrete must wear impervious boots
(gum boots) and rubber gloves.
Eye protection must be worn during wash out of the delivery vehicle.
Workers using vibration equipment must wear earplugs and be taught about vibration white
finger (causes, signs, symptoms and prevention).
Any one exposed to long periods of noisy equipment must wear earplugs.
The supervisor or deputy should be on site during working hours.
Personal protective equipment:
Steel toe safety boots, hardhat and fluorescent vests are mandatory.
All tradesmen to wear PPE correct for his type of work.

Standing rule that any worker, where human life is in danger, can stop the job.
