Here are the following Requirements needed to be submitted before their indicated due dates.
1. Nursing Health History (due on February 11, 2020 not later than 12nn)
2. Functional Health Pattern (due on February 13, 2020 not later than 12nn)
3. Genogram and Ecomap (due on February 12, 2020 not later than 11:59 pm)
4. 3 priority Nursing Care Plan (due on February 13, 2020 not later than 11:59 pm)
5. Case Study (due on February 15, 2020 not later than 12nn)

Guidelines

1. Nursing Health History

In making your health history, make sure that these elements must be followed.

1. Short bond paper, typewritten.

2. Times new roman, font 12, single space, 1-inch margin
3. Each component of nursing history must be present. (pls. see FUCN format)

A. Demographic data
B. Chief Complaint
C. History of Present Illness
D. Past Medical History
E. Current Medications
F. Family History
G. Personal and Social History
H. Review of System
4. Make sure that you get a patient within Foundation University (nursing students are excluded)
or any members of the community (healthcare professionals are excluded).
5. Please make consent for the interview. Kindly include in your consent that you will voice
record the entire conversation.
6. Make sure to submit the document together with the voice memo on or before February 11,
2020 not later than 12nn.
2. Functional Health Pattern

1. Your patient must have a non-communicable disease (Hypertension, Diabetes, Arthritis, and
such).
 2. The Usual Pattern refers to the period wherein the patient was not diagnosed with the
disorder.
3. The initial Pattern refers to the period you are interviewing the patient
4. It would be best if you had an ongoing pattern at least a day only.
5. Make sure each component of Gordon’s Function Health Pattern is present.

3. Genogram and Ecomap

1. Short bond paper, typewritten.
2. Times new roman, font 12, single space, 1-inch margin
3. Make sure to follow the correct format for genogram and ecomap
4. Use the proper and standard legends for both diagrams.
5. For genogram, make your family’s 3 generation genogram
5. For ecomap, create your personal ecomap

4. Nursing Care Plan

1.The patient used for FHP will be the same patient for NCP
2. Strictly, follow guidelines in making an NCP.
3. Make sure to submit the document together with the voice memo on or before February 13,
2020 not later than 11:59 pm)
5. Case Study
1. Refer to your grouping.
2. Patient must have non-communicable disease (DM, HPN, CA and etc.)
3. Pls follow the format
