PMC/PMT remind the contractor to conduct equipment

Contractor is requested to use the new PIR format for any
incident submission.
PMT/PMC reminds contractor to submit their monthly & weekly

statistics reports and tracking records.
Tool box shall be conducted daily prior to start the work.
1.6 PMC/PMT reminding GTH about Fire Safety to including inside GTH Ongoing
the TBT.
PMT/PMC remind GTH on housekeeping to be collected at the

PCH highlight the increasing variance from the revised base

line due to the delay in sub-base and asphalt paving. GTH is

recent stoppages by land owners. Incident reports submitted
