Re: REQUEST TO TRANSFER MEMBERSHIP RIGHTS TO MR. TANADA’S SOLE ACCOUNT

Dear Sir/Madam,

This letter serves to request for a transfer of corporate membership account with STA. ELENA
PROPERTIES, INC. under Omnigrains Trading Corporation (“Omnigrains”) to my sole account,
with respect to the Assignment of Membership Rights Agreement we executed. I hereby state
that I am no longer connected with the aforementioned company. With that being said, I
withdraw all the authorities under my representation on our corporate membership account.
In that case, I would like to request for a transfer of such account into my name, Jose Leonardo
Tanada. Thus, this will now be my personal and sole account. In addition, please note that I will
be paying my monthly dues starting from the month of _________ and the security deposit,
amounting to Php20,000 will be given to Omnigrains. Also, I would like to replace my residence
address as my billing address, specifically _________________________________________.
Kindly confirm upon receipt of this letter.
