LOAN PAYMENT AGREEMENT

This Payment Loan Agreement is made this 15th day of FEBRUARY, 2019.

-between-

GENERIKA PARAISO, INC., a corporation duly registered under existing laws of the Philippines, with
its Head Office located at #2603 Juan Luna St. Gagalangin, Tondo, Manila (1013), represented herein
by its Accounting Officer, Ms. Joselle A. Alfonso hereinafter referred to as the “COMPANY”;

-and-

Ms. Joan F. Montias, Pharmacy Assistant of Generika Paraiso Office, 31 years of age, with address at
72 Seong Villanueva St.,Barangay Ibaba, Malabon City.

WHEREAS, Employee has requested for an SSS Loan and was approved by the company.

NOW THEREFORE, in consideration of the mutual promises contained herein, Company and
Employee agree as follows:

1. Ms. Montias will personally pay for her SSS Loan in the amount of P 13,000, with monthly
payment of 599.88 from January 2019 to December 2020;
2. That Generika Paraiso Inc., will not deduct any loan and/or related charges on her salary;
3. That the company will not be held liable in any problem that may occur in the future.
