2.3. Overturning of vehicle / Crash injuries 2.3.1. All drivers shall have valid driving licenses
2.3.2. The areas where heavy equipment are working shall
be clearly marked on site
2.3.3. Safe access shall be maintained at site for vehicles
and equipment
2.3.4. Use of seatbelts shall be mandatory at all times

2.4. Slips / Trips / Falls 2.4.1. Housekeeping shall be discussed in daily TBT and
be implemented on site on a daily basis

3. Exposure / Protection of existing U/G services 3.1 Damage to COMPANY or existing property 3.1.1. No metal handled tools shall be used on site; any
defect, incident or near miss shall be reported to the
GAMA-HES Manager
3.1.2. No mechanical excavation shall be made nearer
than 1 m of live plant equipment or existing underground
utilities
3.1.3. Location of U/G services shall be identified by a
cable detector operated by a competent person
3.1.4. U/G services shall be marked on site; and they shall
be barricaded with steel pegs at each side
3.1.5. The safe work methods and hazards associated with
tasks shall be covered in daily TBT
3.1.6. Operators shall be informed about the locations of
existing facilities, which shall be clearly marked on site
3.1.7. A trained and capable lookout shall be assigned
where digging is in progress
3.1.8. The existence and locations of U/G services shall be
reported to the Construction Manager when uncovered,
who shall inform Technimont
3.2. Injuries due to collisions or unstable material 3.2.1. First Aid kits and certified First Aiders shall be
available on site clinic
3.2.2. All employees shall be made aware of Emergency
Procedures
3.2.3. Audible reversing alarms shall be installed on each
vehicle
3.2.4. Entry to the excavation area, which shall be properly
barricaded, shall be prevented when excavation is in
progress
3.2.5. Excavated material shall be stocked at least 1.5-m
away from the edge of the excavation
3.2.6. Side slopes of excavations should be stable

3.3. Unsupported services 3.3.1. Existing facilities / structures shall be supported as


per the instructions from competent person, if required

3.4. Permit to Work 3.4.1. The required Permits to Work shall be obtained and
kept at work area at all times along with the JSA

3.5. Dust Control 3.5.1. Provision of dust masks for workers and ensure
their use
3.5.2. Provide dust mitigation by spreading water
3.5.3. The distance from the excavator bucket to the body
of the truck shall be reduced to an acceptable height (0.75
to 1 m)

4. Excavation Works (Excavator & Truck) 4.1. Night shift 4.1.1. Although none foreseen, should night shift be
required, necessary precautions shall be taken (they shall
be discussed on a separate JSA if required)

4.2. Collisions / equipment falling into the 4.2.1. No equipment shall work nearer than 1.5 times the
excavation excavation depth from the excavation
4.2.2. Banks-men with high-visibility vest shall accompany
the activities

4.3. Persons under the bucket 4.3.1. No persons shall be allowed to stand under the
bucket of the excavator

4.4. Lifting with the bucket 4.4.1. No lifting shall be made using the backhoe

4.5. Passengers on operating equipment 4.5.1. No passengers are allowed to ride on a single
designed cab

4.6. Resting under equipment 4.6.1. Resting under the equipment is prohibited

4.7. Unauthorized equipment operator 4.7.1. Only competent and certified operator shall operate
the equipment, when left unattended, the engine shall be
switched off and the keys removed from the ignition
5. Loading, transportation and Topsoil removal 5.1. Material falling from trucks 5.1.1. All materials on the trucks should be effectively
secured during transportation
5.1.2. Operators and Job Supervisor shall ensure that the
vehicle is not overloaded
5.1.3. All trucks shall be equipped with covering canvas
with ropes for easy (un)covering

5.2. Speeding 5.2.1. Operators shall be made aware of traffic rules and
regulations; penalty system shall be implemented if
required

5.3. Excavated Material 5.3.1. Excavated material shall be placed at least 1.0m
away from the excavation

5.4. Operator struck by the bucket 5.4.1. Truck driver shall leave his vehicle and stand at a
safe distance during loading

6. Cutting of Side Walls 6.1. Collapse of side walls 6.1.1. Slopes shall be stable (AS PER CLIENT
STANDARD)
6.2. Ingress / Egress 6.2.1. Sufficient ramps and stairs shall be provided for
ingress / egress
6.3. Lack of inspection 6.3.1. Excavations shall be inspected daily by a competent
person and same inspections shall be documented

7. Entrance to the trench to execute the work 7.1. Access across trenches 7.1.1. Provide access roads across the trenches

8. Demobilization 8.1. Work completion 8.1.1. Workforce shall be alerted about the work
completion
8.2. Clutter/Rubbish 8.2.1. Good housekeeping practice shall be implemented
throughout the day
