MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This MEMORANDUM OF AGREEMENT is made, executed and entered into by:

The, with principal business address herein represented by, of legal age, Filipino, single/married
and a resident of Barangay ______, Tagbilaran City, Bohol, Philippines, hereinafter referred to as the
FIRST PARTY;

The herein represented by its head of office, and hereinafter referred to as EMS PROVIDER and
as the SECOND PARTY;

W I T N E S S E T H:

WHEREAS, EMS PROVIDER, THE CITY HEALTH OFFICE OF THE CITY

GOVERNMENT OF TAGBILARAN, BOHOL must enter into a Memorandum of Agreement
with the SKILL’S ENHANCEMENT AND DEVELOPMENT TRAINING CENTER to provide
ambulances, air medical transportation, personnel and equipment in response to a public health
emergency. This document is a binding contract as well as guideline for operations.

I. Introduction: There is a recognizable need to coordinate EMS Operations in the

event of a natural or man-made disaster or emergency circumstances of
students of Skill’s Enhancement and Development Training Center .

public health emergency. The City Health Unit may provide appropriate
training for the administration necessary for the adequate
protection and performance of the Center personnel.

III. Goals of Medical Support Activity:

1. Enhance patient, medical treatment, transport, and transfer of
care during emergencies.
2. Develop and/or enhance regional transfer/transport agreements.
3. Reduce the likelihood of death, and their injury, and/or illness and
their related effects to
Response personnel, victims, and bystanders.

4. Enhance the working relationship between the Training Center and the
City Health Unit
2. Acquire and maintain communication systems that are interoperable
between
the training center and the health unit.
3. EMS Providers unable to dedicate ambulances for a regional or state
response
must make available supplies and/or equipment in accordance with the
HHS
Emergency Preparedness Grant Funds for a regional or state response.
An addendum or waiver to the Memorandum of Agreement (MOA)
outlining
the
conditions is required to be eligible for HHS Emergency Preparedness
Grant Funds.
4.Rescue immediately in case of emergency.

V. Responsibilities of the Training Center

1. Guidance: Provide technical assistance to ensure that policies and

procedure
of the health unit are followed appropriately.

2. Training: Attend training opportunities given by the EMS service
providers.

The principles and policies of this memorandum of agreement are

hereby
understood and agreed upon by the Training Center and EMS
Provider
indicated below.
This will amend any previous MOU/MOA and will remain in
effect from the date of the signature by both parties unless
modified by the Training Center by written notice to the EMS
Provider.
