Universal Design for Learning (UDL): Accessing Formative is very easy if users have a device such as a
computer, lab top, tablet or phone that has working internet. Students don’t have to create an account; they
can use a link to access the assessment. Students are supposed to work individually to complete the quiz. I
have text for those who have any form of auditory impairment. There are no questions that require a video or
picture and students can use a text to speech app to have the questions and answer choices read aloud for
those who are visually impaired.
Describe the instructional activities that will occur PRIOR to the SRT activity and how you will introduce
the SRT activity. I would start with a Nutrition review game like jeopardy where students can work together
as a team and answer review questions relating to Nutrition. Once the review game is over, students will go
back to their seats and each get a device (Ipad, computer, lab top, phone). I will introduce the assessment
tool by starting off and saying good job to everyone who participated in the review game. Now that we have
done a review in teams, it’s now time to test your own knowledge through a tool called Formative. Now,
everyone type or click on this link and it should bring you to your “Nutrition Assessment”.

Briefly describe what will happen DURING the SRT activity: Students will need a device that has working
internet such as a tablet, lab top or computer. Once students have accessed the link, they will take the 10
question assessment quiz which is self-paced. As students are going through the assessment, they will see on
the board that there is a leaderboard. The leader board represents the highest amount of points for the users
that are participating. To gain points, you have to answers the questions correct and as quickly as you can to
get the most points. If students have a question, they are more than willing to raise their hand or come up to

my desk and ask me. The Activity should last no more than 10-15 minutes. I will walk around to make sure
students questions are answered and that they are doing their own individual work.

Why or why not? I will allow the correct answer to be shown after answering the question so that students
know what the correct answer is in the future.
Describe what will happen AFTER the SRT activity? After the assessment, I will start a discussion by
reviewing each of the questions. Based on the percentage of correct answers, I will need to spend less/more
time on reviewing each question. I will ask students what was most challenging and give them way to
overcome that challenge. I will let students know that their score should matter and more about how honest
their self-assessment was. I will ask students where do they think they need to go next as far as Nutrition goes
and how we can set more learning goals.
How will the data be used? The data that is collected reflects each students’ individual results. Students as
well as the teacher can go back and see specifically what questions they have missed. Students can ask
questions and I can review with the class about each of the questions. I will collect these grade percentages as
a quiz grade. I can show/share their progress with their parents to gauge where their students are. I could put
students into equal groups based on how their performance was when doing another review game.

Describe your personal learning goal for this activity.


I hope that student can learn nutrition through a different technology tool. Another goal of this activity is to
get students learning the importance of nutrition and having them think of ways for them to included a
healthy balanced diet including the 5 healthy food groups. I hope students like that there a variety of ways to
answer questions and that the answers are personal to them and their own lifestyle. This will help teachers
learn to use many, different technology tools to help assess their student’s knowledge.

Reflective Practice: After designing this lesson idea, hopefully I and other teachers around the country can
implement this assessment in their health class especially in their nutrition unit. Hopefully students can
acknowledge that taking assessment isn’t about the grade but realizing what they can take away from this
lesson. I designed this lesson idea at the end of a unit, but you can also incorporate it at the beginning of your
nutrition unit to see where students prior knowledge is at. To further extend this activity, you could add more
questions to the assessment or have students each create one question to add to the assessment.
