“RECOMMENDING TO HONORABLE MAYOR EVAN HOPE D. OLVIS THE DESIGNATION

OF ________________________________________________, LOCATED AT PUROK _______,
_____________________, POLANCO, ZAMBOANGA DEL NORTE AS BARANGAY
ISOLATION UNIT (BIU) FOR PURPOSES OF COVID-19 CONTAINMENT AND CONTROL

WHEREAS, the spread of Corona Virus Infectious Disease (COVID-19) in considerable

number of countries on an international scale at present is extremely alarming;

WHEREAS, Philippines is not an exemption to this case, and in fact there are already a few
individuals in the National Capital Region who are confirmed positive with COVID-19 infection;

WHEREAS, all local government units are enjoined by the national government thru DILG
Memorandum Circular to take active response to the growing problems relevant to the spread of
COVID-19;

WHEREAS, the same memorandum directs all Punong Barangay to recommend to the
Municipal Mayor an enclosed structure, such as an empty house or building to be designated as
Barangay Isolation Unit (BIU) for the containment and control of the spread of COVID-19 infection;

WHEREFORE, upon motion of Barangay Kagawad _______________________________,

RESOLVED as this body hereby resolves to recommend to Honorable Mayor Evan Hope D.
Olvis the designation of ________________________________________________, located at Purok
_______, _____________________, Polanco, Zamboanga del Norte as Barangay Isolation Unit (BIU)
for purposes of COVID-19 containment and control;

RESOLVED FURTHER to furnish a copy of this memorandum to Hon. Mayor Evan Hope D.
Olvis, for his appropriate action in accordance with DILG Memorandum Circular 2020-023;

RESOLVED FURTHER to furnish copies of this resolution to the MHO, DILG and all other
offices concerned for their information and guidance.
