Biographical Data

Mrs. E.M, female, Filipino, is residing at Tambo, Paranaque City. She was born on
November 08, 1941 at their home in Mindoro. She is a Roman Catholic and married to
B.M. According to her son, her current occupation is “labandera” in their barangay. She
has three children. The source of reliability is her son namely P.M. she was admitted last
November 28, 2010 at around 2:00pm. Her physician is Dr. Lee.

She was interviewed last Monday, December 06, 2010 at around 9:00am by Kevin Bah
Lim and Trissian Gayle Aldon.

Chief Complaint

“Nanghihina at sumakit ang ulo at katawan nya medyo nahirapan din sya huminga” as
verbalized by her son Mr. P.M.

Medical Diagnosis: CVA

Present History

Prior to admission the patient experienced weakness, headache and difficulty of


breathing, she was at their house at that time, and washing some clothes, because she is a
“labandera” in their barangay. Her son put the client on bed and gave Paracetamol
500mg. But after a few hours, the client decided to be confined in the hospital at
Florencio V. Bernabe Sr. Hospital, for check up and for them to following her condition.
The doctor’s diagnosis is CVA.

Past History

Patient is known hypertensive since February 2001, non diabetic. She is not
taking any hypertensive drug previously. She was diagnosed to have asthma 10 months
ago. Among other illnesses by the patient are fever, cough and colds.

The patient has not undergone any surgery yet. She received some immunization
but her son doesn’t know or can’t innumerate those vaccines given. She do not have any
food or drug allergies, but her skin is sensitive to some detergents. She is not an alcoholic
drinker and she do not smoke according to her son.

Personal and Social

She resides with her family at Tambo, ParanaqueCity. She is a high school graduate, she
was not able to enter college because of financial constraints. Her recent oocupation and
her source of income is being a “labandera” in their barangay. Her occupation serves as
her daily activities and after washing some clothes, she spend time to her husband and
other members of the family. Her form of exercise includes stretching and walking 2-3
times a day. She is not an alcoholic drinker and not known smoker. She eats 3-4 times a
day. She seldomly attends or visit health center/clinics because of her work.
