II. Project Summary:

Hygiene and Sanitation Education is one of the top priorities of San


Rafael National and Vocational High School because we believe that health is
inextricably linked to educational achievements, quality of life, and economic
productivity. By acquiring health-related knowledge, values, skills, and
practices, students can be empowered to pursue a healthy life and to work as
agents of change for the health of their communities.
School is the next place after home, where students spend their
maximum time and a place like a school is very vulnerable to transmission of
infections. This is the reason why we take advantage of any and every
opportunity to provide our students adequate information on good personal
hygiene practices to prevent the spread of disease and maintain good health.
Project PHASE was conceptualized to help San Rafael National and
Vocational High School students know the importance of good personal
hygiene, and help them manage their own personal hygiene routines, and to
protect the health of the school community.

To realize this goal, speakers from the City Health Office (Nurses,
Doctors, Barangay Nutrition Scholars, Health Welfare Personnel) will be invited
to conduct symposium and health awareness campaign in the school.
Distribution of hygiene and first aid kits, posting of information and advocacy
materials in strategic places, renovation of the school canteen, supplemental
feeding to undernourished students, repairing our water facilities for adequate
water, delivery of health services such as nutritional supplementation,
providing creative activities and recreational experience (Zumba, games, sports,
board games), establishment of menstruation area for students, and improving
the school clinic are the activities to be undertaken to make this project become
successful.
Project Phase will start on February 2019 and ends on May 31, 2019 (4
months) and will be implemented at San Rafael National and Vocational High
School. The project cost is______________.

III. Project Background

Poor personal hygiene contributes to cross transmission of


microorganisms, gum infections, increased rate of infectious illnesses, incidence
of food borne outbreaks and reproductive tract infections. These have been
attributed to inadequate knowledge of personal hygiene and its practices.
Based on a survey conducted to grade 7 students, it was found out that
most of them heard about personal hygiene but only 85% of the Grade 7
students of San Rafael National and Vocational High School followed good
hygiene practices. It is for this reason that Project PHASE was initiated to
address the students’ inadequate knowledge of personal hygiene by providing
them awareness and preventive health care, help them manage and practice
their own personal hygiene routines, to protect the school community, and
prevent the spread of disease.
Since good personal hygiene is essential to promoting good health, the
school will be inviting speakers to provide the whole school community specially
the students information about general hygienic measures to help them live a
healthy lifestyle at home and in school.

IV. Project Objectives

1. To protect the health of the school community by providing students,


parents, faculty and staff information on awareness and preventive
health care.
2. To provide free hygiene and sanitation kit to every classroom.
3. To help students manage their own personal hygiene routines.

B. Project Deliverables

1. Hygiene kit – provision of soap and other hygiene consumables to


support the personal hygiene routines of students.
2. Hand hygiene education – teachers can incorporate hand hygiene
education into the curriculum and daily school activities to
maximize opportunities for students to develop personal hygiene
practices.

3. Personal hygiene care and learning plan – step-by-step processes
of hand hygiene, menstruation management etc.
4. First aid kit/medicine cabinet – necessary to treat minor ailments
and injuries that happen in school and to help reduce the risk of
infection or the severity of the injury.

C. Project Risk Management

1. Resource Shortfalls

The project team may lack resources for the implementation of the
program. In this regard, the team may seek assistance from the stakeholders
specifically parents.

2. Availability of Resource Speakers

To assure the availability of facilitators a request letter will be forwarded


ahead of time to the different offices. (i.e. Nurses, dietician, doctor from the City
Health Office II)

3. Students might feel embarrassed discussing personal hygiene

To make sure that students do not feel embarrassed discussing this


topic, we should maintain professional boundaries with some sensitive
subjects. (example: using pads, putting on deodorants and others.)

4. Distribution of hygiene
kit

The financial needs of the school for the implementation of the project will come
from donations of the stakeholders.

VII. Monitoring and Evaluation

Monitoring the success of the project is important to ensure that the


project is implemented as planned. It is vital to identify which indicators are to
be measured. These are to be noted and communicated to all team members
and stakeholders. A frequency of reporting as well as a format needs to be
decided upon and clearly communicated to all those who will be expected to
issue reports. If the monitoring framework is clearly defined, then there may not
be any need for huge amounts of data collection. Too much irrelevant data will
only create confusion and add no value. Quality of data to ensure relevance
needs to be the focus of any data collection efforts. Selection of appropriate
tools for data collection is also important. A wide variety can be used including
questionnaires, surveys, checklists and focus group discussions. It is also
pertinent to assign a specific person for each type of reporting or monitoring
activity. Those tagged with reporting should be told clearly whom they are to
report to.
To monitor and evaluate the success of the project, any of the following
monitoring tools and techniques will be employed: Questionnaires or surveys,
feedback forms, interviews and focus group.

VIII. Next Steps

1. Monitoring of Team Performance – we need to monitor how
everything is going, including what the team is working on, which team
members are stuck on a task or what other tasks need to be done to move
forward with the project.
2. Regular status and progress report – it is important to know how
much more is needed to complete the entire project so we can identify existing
issues and make timely adjustments to get things back on track.
3. Providing recommendations and suggestions – we have to estimate
the percentage of project success or failure and provide valuable advice on how
to get the team to work together to prevent missing the deadline.
4. Ensuring that recommended actions are implemented – it is
important that the team is clear if there are any changes on the project plan.
We can remind the project team of any hanging tasks and issues that need
quick resolutions so we can drive the team to the direction it needs to go.
