HSE Recognition and Disciplinary Guideline

1.0 Purpose
The purpose of this guideline is to ensure consistent implementation of the Sumitomo’s
Cardinal Rules and consistent application of appreciation for excellent HSE performance,
disciplinary actions against the rules breaches or any other HSE violations within Central
Java Power Project.

2.0 Scope
2.1 This procedure applies to the Central Java Power Project, including all subcontractors that
involved in the project.
2.2 Recognition or reward could be given to a worker based on his/her positive attitudes, influence
behaviour and/or excellent performance towards HSE and Sumitomo’s Cardinal Rules.
2.3 Disciplinary action could be charged to a worker as consequences of his/her violation. The
level of discipline is progressive, increasing with the frequency and/or severity of unacceptable
performance regarding Sumitomo’s Cardinal Rules.

3.0 Definition
3.1 Sumitomo’s Cardinal rules are the life-saving principles that have been identified for this project.
Failure to adhere to these rules by any employee or employee of a Sub Contractor or sub-sub-
contractor will be considered a serious transgression. These rules are being implemented to
prevent serious injury or death of any employee or contractor working in any area within the
site.
3.2 At-risk situation is unsafe acts and/or unsafe conditions, a deviation or non-compliance to
standard procedures, safety policies and regulations that has the potential to cause harm to
the health and safety of personnel and/or to the environment and/or material resources.

4.0 HSE Recognition Guideline
4.1 HSE Recognition will be awarded to any worker that has excellence contribution in preventing
incidents on the project.
4.2 The reward will be given in the events of Daily Tool Box Meeting, Weekly Patrol/Walkdown,
and Mass Monthly Meeting.
4.3 Guideline for those rewards is as follows :

4.3.1 Reward on Daily Tool Box Meeting

4.3.3 Reward on Mass Monthly Meeting

RULE DESCRIPTION OF RULE
Stop, Think, Act, Review
Rule 1 A risk assessment shall be conducted prior to every job being carried out. (First stop and think;
then act. On completion of the action, review the process.)
Hook up at Heights
Any person who performs work higher than two metres above ground level shall be attached
Rule 2 to an anchor point at all times, or as identified during the risk assessment. Only safety
harnesses or double lanyard harnesses is allowed. Safety belts are prohibited. Lifelines will
comply to International standards.
Fall Protection
Rule 3 Temporary fall protection shall be of a rigid material to resist weight of at least 100 Kg at any
location higher than 1200mm. This includes Excavations.
Electrical Equipment
The permit to work system shall be adhered to at all times. Any person who performs work on
an electrical installation shall ensure that it is isolated, tested and earthed before starting with

any work. Temporary switch gear, leads and tools shall be maintained and controlled with a
regular checklist. Access to generators shall be strictly controlled to prevent unauthorized
entrance. ELCB shall be installed in Panel Box.
Scaffolding and Ladders
Scaffolding and Ladders will be erected and maintained continuously according to
Rule 5
International requirements. No homemade equipment shall be allowed unless certified and
labelled safe by the construction manager.
Excavations
Rule 6
Excavations will conform to International standards without exeptions to ensure stability.
Stacking and Storage
Safe and neat stacking and storage practices will be applied in NORTH/SOUTH or
Rule 7
EAST/WEST directions for building and construction material. No protruding nails or sharp
objects will be tolerated.
Housekeeping, Hazardous Substances and Hygiene
Work/rest areas will be maintained neat and tidy, and toilet facilities will be kept clean and tidy

and serviced on a daily basis. Hazardous substances will be managed according to regulations
and spillage on site prevented at all cost.
Confined spaces
Rule 9
Confined spaces will be managed and controlled according to International standards.
Buckle up/ Sober up
Rule 10 Seatbelts shall be used at all times while driving to work and home.
No person is allowed to be under the influence of alcohol or drugs while on duty.
PPE
Rule 11
Required PPE shall be used as prescribed in Inductions and Risk Assessments.
No Transporting of People on Trucks
Rule 12
Under no circumstances will any person be allowed to be transported on the back of any truck.
Moving heavy equipment
Rule 13 When driving, moving and/or operating heaving equipment, no personnel shall be allowed to
“sit or ride” on the equipment except for the certified operator.
Diving Activities
Rule 14
Only certified and trained worker that be allowed to do diving work
No Speeding On Site
Rule 15
All vehicles shall obey site speed limit rules at all times.

Non-compliance to a cardinal rule will be considered as a serious violation and will lead to
disciplinary action which may result in immediate and permanent dismissal from the project.
