to participate. If you decide to participate, you will be asked to sign this consent form. After
you sign this consent form, you are still free to withdraw at any time and without giving a
reason. If you withdraw from the study before data collection is completed, your data will be
destroyed.

entitled: “An inquiry on why FEU college students continue to smoke cigarettes
despite increment increase in price every year” as part of our requirements in
GED0112 Scholarly Inquiry.

As we look for the participants, you are being qualified for the criteria which we have
provided. In line with that we are inviting you to an interview to be held at the Far
Eastern University - Manila depending on your time availability (you may choose
from the given date and time below)

interview will be good for at least one (1) hour. Your responses to the questions will
be kept confidential.

There is no compensation for participating in this study. However, your participation
