INFORMED CONSENT FORM

FOR RESEARCH PARTICIPANTS

Title: Challenges Encountered in Teaching Physical Education in times of Pandemic

Research description: This research study, to be conducted by Maricar C. Tegero, will reveal the
challenges encountered in teaching Physical Education amidst pandemic. Selected PE teachers will be
interviewed to understand the phenomenon and come up with interventions or effective solutions to
address those challenges. You are invited to join this research study because you meet the selection
criteria.
Privacy & confidentiality: Private information collected in this study will not be shared with the public.
The names of the participants as well as where they live will not be disclosed to anyone during or after
the completion of this study. All information that can reveal the identity of the research participants will
systematically be removed from the data before the data analysis starts. To do so, each research
participant will be assigned an anonymous identifier or a letter.

Voluntary participation: Research participants freely volunteer to participate in this research study,
without being forced. They have the freedom to share what they want to share to stop participating if they
want to stop at any time.
Risk: There is no known emotional or physical harm that this research study can cause to the researchers
and the research participants of this study.
Audio recorder: The researcher will be using an audio-recording device but will keep information
confidential.
Honesty: Both the researcher and the research participants agree to be honest in both data collection and
reporting. No dishonesty is allowed.
This is to certify the undersigned adhere to the following:

 I clearly understand the nature and purpose of this research paper.


 I participate voluntarily in this research project.
 I give permission for my speech (or interview) to be audio- (and/or video-) recorded.
 I understand that details about my identity, as well as recordings of my speech (or interview) will be
stored for 30 days and discarded afterwards.
 I understand that my anonymity will be ensured and that details about my identity will be kept
confidential.
 I agree to have extracts of transcripts from recordings of my speech (or interview) quoted in a
research report and/or publication.
 I understand that I may withdraw my consent within two days before the scheduled date of data-
gathering (recording).

INTERVIEW GUIDE

Title: Challenges Encountered in Teaching Physical Education in times of Pandemic

A. Before the interview proper


1. Arrive on time/ be online at least 2 audio-recorders, a pen/pencil, and a notepad.
2. Have the informed consent form ready.
3. Thank the participant for accepting your invitation to be part of the study.
4. Go over the informed Consent Form
5. Address any question the participant may have.
6. Have the participant and yourself sign the form.
7. Inform the participant that you are going to start recording now.

B. During the interview


1. How many years are you in teaching PE?
2. What is your highest educational attainment?
3. What are the problems that you encounter in teaching Physical Education classes
during the pandemic?
4. What are your reactions on the teaching/ online modalities/platforms used in
teaching?
5. What kind of teaching do you experience in your online classes?
6. As you encounter the challenges in online classes, how do you manage to solve them?
7. Amidst this online teaching-learning context, how do you intervene and solve these
challenges?
8. What support do you think could possibly be extended in your problems relative to
teaching PE in the midst of pandemic?

C. Ending the Interview

1. We are about just to finish. What other important information pertaining to this topic
can you share with me?
2. Thank the participant.
3. Inform the participant that you might contact him/her again in the near future if there
is something you need additional information on.

D. After the interview


1. Make sure at least 1 audio recorder recorded well.
2. Leave the research setting in the same (organized) state you found it.
3. Find a quiet place where you can write more elaborated field notes and/ or analytic
notes.
