AFFIDAVIT OF ADVERSE CLAIM

3. That when I requested the Register of Deeds for the Province of

SALE at the Memorandum of Encumbrances of said title, I
learned that a new title, Transfer Certificate of Title No.

4. That in view of the foregoing premises, I hereby request the

Register of Deeds for the Province of Pangasinan to cause the
entry of this affidavit at the Memorandum of Encumbrances of
