Additional Requirements for the Real Property Contribution

1) Matrix of Properties - location, area, TCT/CCT, tax declaration, basis of the transfer
value
2) Latest BIR Zonal Value - certified by BIR, if transfer value is based on zonal value
3) Appraisal Report - authorized by appraiser, if transfer value is based on appraised
value (not more than 6 months old)
4) Deed of Assignment
5) Mortgage or Creditor's Certification - if property is mortgaged, based on the
outsanding loan balance and written consent to the transfer of the property
6) Lease Contract and Consent of Landowner to the Transfer - for assignment of
building where assignor is not the owner of the land
7) Affidavit to Change Name - by an incorporator of the corporation to submit the proof
of transfer to the corporation within the prescribed period
8) Certified True Copy of TCT or CCT and Tax Declaration Sheets 
9) Required Document(s) for Filipino Board Member
