Directions: In light of the value of HONESTY, you are to accomplish the form based on your accomplishment of the assigned
Written Works and Performance Tasks. Using the information in your email sent box, fill in the columns with the appropriate data.
Use the list of Written Works and Performance Tasks, due date, type of output, and maximum points in this form. Put a check mark, if
the submission is a Written Work or Performance Task and if it is on time (with the first date after the date and time assigned) or late
(second day after the date and time assigned).
In addition, you need to provide your own holistic rating (points) on your own works and tasks. It will be added to the Tutor’s rating.
Do not forget to add separately the total points for the Written Works and Performance Tasks.
Moreover, if you were awarded exemptions, you must reflect the number of exemptions given to you in any given time and activity.
Kindly, inform the Tutor of your participation in the Kahoot activity and the number of participations you did.
Finally, you can still submit Written Works and Performance Tasks until May 7, 2020 (Thursday). After May 7, any unsubmitted
works or tasks will be considered unmarked (Zero).
