Activity 12 (5 points)

Direction: Interview at least two (2) MAPEH teachers on how they do the computation
for the 4 learning areas of MAPEH (Music, Arts, PE and Health). Attach the documentation
of your interview here.

Activity 13 (5 points)

Direction: Explain in your own words on how do you understand the following in the
application of formative test to students and teacher:
1. Assessment for Learning
Teachers incorporate data about students' abilities, knowledge, and
comprehension into their education. It is commonly used to clarify students' knowledge
and learning at various points during the teaching and learning process, and is also
referred to as "formative assessment." Assessment for learning is a formative assessment
technique that emphasizes using assessment to promote and improve student learning.
This form of evaluation is utilized during the learning process to offer feedback and assist
students in identifying areas where they may want further assistance or practice. Learning
assessment is frequently informal, continuous, and integrated into learning activities,
helping students to better grasp what they are learning and how they are growing.

2. Assessment as Learning
Occurs when pupils act as their own judges. Students maintain track of their
own learning, ask questions, and utilize a variety of approaches to figure out what they
know and can accomplish, as well as how to use evaluations to help them learn new

things. Assessment as learning is a formative assessment technique that focuses on

utilizing testing to help students evaluate their own learning and build their metacognitive
abilities. Students are actively involved in the evaluation process under this method, and
they are encouraged to reflect on their own learning and development. Students can gain
a better grasp of the learning process, discover their own strengths and limitations, and
build learning techniques that are personalized to their own requirements by
participating in this sort of evaluation.
Direction: Show a step-by-step computation of grades (from step 1 to step using the
