AFFIDAVIT OF UNDERTAKING

I, ARCHIE Z. TAN, of legal age, married, Filipino and a resident of M.H. Del Pilar,
Molo, Iloilo City, after having been duly sworn in accordance with law, do hereby
depose and state:

That I am the owner of Crown Agrikem Corp. having a branch at Recto St., Ilaud,
Poblacion, Iloilo;

That currently, I am processing my business permit in the Business Permit Licensing

Office (BPLO) of LGU Barotac Nuevo;

That I undertake that I shall allow only load and unload passengers who are my
customers in front of my establishment;

That I likewise undertake to follow strictly the “no parking” rule of the Municipality;

That in case of violation of any of the foregoing, I interpose no objection to the

automatic suspension, cancellation or revocation, as the case may be, of my business
permit and license, and the subsequent closure of the above-cited business
establishment, if necessary; and

That I execute this affidavit in order to attest the truth of the above facts for all legal
intents and purposes it may serve;
