AFFIDAVIT FOR LATE REGISTRATION OF BIRTH

I, ANTONIO MARTIN ICHON, of legal age, married to Filomena

Glipa Ichon, and with residence and postal address at Brgy. Luz, Cebu City,
after having been duly sworn to in accordance with law, do hereby depose
and say:

1. That I am the applicant for the delayed registration of my birth;

2. That I was born on January 30, 1953 at Malitbog, Leyte to

spouses
Bienvenido Ichon and Rosario Martin;

3. My parents got married on May 6, 1951 at Malitbog, Leyte;

4. That the reason for the delay in registering my birth was due to
inadvertence on the part of my parents and the prevailing
conditions then, my birth was not reported and registered with the
office of the Municipal Civil Registrar of Malitbog, Leyte immediately
after birth;

5. That a copy of my birth certificate is needed for the purpose of

claiming my senior citizens benefits and in processing of my
children’s requirements for overseas employment;

6. That this affidavit is executed to support my application for late

registration of birth and for whatever legal purpose this may serve
