CONSENT FORM

I voluntarily agree to participate in this interview for the purpose of improving the
research project of Senior High School students of La Salle College Antipolo about The
Experiences of the SHS Students of La Salle College Antipolo on the Effects of
Online Advertisements. I understand that this interview is being conducted by Eric
Garcia, Justine Ungab, Phyrra Balajadia, Marianne Monton, and Eunice Owebar to
improve the study that they are conducting. I understand that the interview methods
which may involve me are:

1. the students/interviewer’s recorded interview of my answers to the interview

questions, and/or

2. my completion of interview question(s), and/or

3. my participation in a 30-60 minute interview.

I grant permission for the interview to be tape recorded and transcribed, and to be used
only by the above mentioned interviewer/s for analysis of interview data. I grant
permission for the evaluation of the data gathered from the above methods to be
published.

I understand that any identifiable information in regard to my name and/or agency name
may be listed only in the above-mentioned interview.
