REVOCATION OF SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

I, APRIL JOY FERNANDEZ, of legal age, Filipino, and a resident of 2214B East
Aguinaldo Avenue, Clark Freeport Zone, 2023, Pampanga, Philippines, do hereby revoke the
Special Power of Attorney dated March 1, 2017 and recorded in the Notarial Books of Atty.
Mike Ross, Notary Public for the City of Angeles as Doc. No. 230, Page No. 72, Book No. L,
Series of 2017, I granted unto GILBERT ENRIQUEZ, of legal age, Filipino, and with postal
address at 2214B East Aguinaldo Avenue, Clark Freeport Zone, 2023, Pampanga, Philippines to
COVER ALL ISSUES RELATING TO MY HEALTHCARE when incapacitated due to sickness
or injury, with the power to make such decisions and give informed consent in my behalf and
relay the same to any medical doctor, practitioner or hospital and, in case of my death, to have
my body discharged from the hospital and repatriate my remains repatriated to the United
States of America.

NOW THEREFORE, by virtues of these presents, hereby REVOKE, ANNUL and

AVOID the said power of attorney and all powers and authority therein or thereby given or
granted, or intended to be given or granted to said GILBERT ENRIQUEZ.

IN WITNESS WHEREOF, the undersigned has set his hand on this 17th day of
February, 2020 at Angeles City.

ACKNOWLEDGEMENT

BEFORE ME, a Notary Public for and in the City of Angeles and the Province of
Pampanga personally appeared APRIL JOY FERNANDEZ, presenting to me her Philippine
Passport No. 123456 valid until Feb. 27, 2022 known to me and by me known to be the same
person who executed the foregoing instrument and acknowledged that the same is his own
free, voluntary act, will and deed.

The foregoing instrument relates to a Revocation of Special Power of Attorney and

consists of One (1) page, including this page on which this acknowledgement is written, signed
by the grantor and their witnesses on each and every page thereof.
