PERSONAL AFFIDAVIT

I, MARIA ANCILLA G. ALCOPRA of legal age, Filipino,single and a resident of Balangasan Pagadian
City Zzmboanga del Sur,Phillipines, after having been duly sworn in accordance with law, do herby
depose and say;

That I am the same MARIA ANCILLA G. ALCOPRA who was born on March 7,1959 at
Sinacaban, ,Misamis Occidental;

That since I started school I used the date of Birth as March 7,1959 since the school Registrar
before did not require to present the Certificate of live birth and the PBET before was not
meticulous about the correct entries of certificate of live birth and correction was made only after
a lapse of time due to negligence.

That I did not file any petition of correction of my Date of Birth in the local Civil Registrar.

That in my PRC record my Date of Birth appeared as March 21, 1959 Instead of the date of
Birth as of March 7, 1959 as reflected certificate of live Birth;

That I am executing this affidavit to attest the truth of foregoing facts;
