AFFIDAVIT OF GUARDIANSHIP

I, NARCISA G. IBAG, of legal age, Filipino and a resident of Bgy. Sicsican,

Puerto Princesa City, after having been duly sworn to in accordance with law hereby
depose and state that:

That I am the grandmother and guardian of JOHN LLOYD LAMANDA IBAG

for he is under my care and custody since his parents are residing outside Puerto
Princesa City;

That he is a student of San Pedro Elementary School for the School year
2019-2020;

That on August 19, 2019, the said school requires the submission of my
grandson’s Birth Certificate duly authenticated by the Philippine Statistics Authority
(PSA);

That said PSA Birth Certificate is not intended for any illegal of unlawful
purpose but solely due to the foregoing reason;

That I am executing this affidavit to attest to the truth and veracity of the
foregoing statements and to comply with the requirements of the law.
