AFFIDAVIT

I, ROSEMARIE A. LIGAN, Filipino, of legal age, married and a resident of

South Mapang, Rizal, Zamboanga del Norte, after having been sworn to an oath in
accordance with law, do hereby declare:

That I am a permanent employee of the Local Government Unit of Rizal holding

an item as Sanitation Inspector 1;

That I am applying for retirement;

That I am giving authorization to deduct all of my financial obligations with the

LGU of Rizal from the benefits due to me;

That there is no pending criminal or administrative case or investigation against

me;

That, I am executing this sworn affidavit in good faith, freely and voluntarily and
to attest to the truthfulness of the foregoing statement, for whatever purpose it may
serve.
