Modified Road Test Agreement

1. Applicant declaration (initial each statement)

a) I will not communicate verbally, or in any other manner, with my supervising driver unless I am required to communicate for safety reasons.

b) I understand that communication with my supervising driver, other than for safety reasons, will result in a failed road test.

c) I understand that SGI will not provide copies of the road test video footage to myself or my supervising driver.

d) I understand that my road test result (pass/fail) will be communicated to me by phone or email within one business day.

e) I understand that I am not permitted to enter an SGI facility during the testing appointment.

3. Supervising driver declaration (initial each statement)

a) I meet the eligibility criteria to be a supervising driver:

Class 5 road test

1. I hold a Class 1, 2, 3, 4 or 5 driver’s licence and have held that licence for at least 365 days in the preceding three years.
2. I am not a novice driver.
3. I am capable of lawfully assuming control of the vehicle if deemed necessary.
4. I live in the same household as the applicant.

Or

Commercial road test

1. I hold the class of licence, or higher, for which the applicant is testing and have held that licence for at least 365 days in the
preceding three years.
2. I am capable of lawfully assuming control of the vehicle if deemed necessary.

b) I will sit in the front passenger seat of the vehicle.

c)  I understand I may be filmed during the road test.

d) I will not communicate verbally, or in any other manner, with the applicant unless it is to intervene in an emergency.

e) I understand that communicating with the applicant, other than for safety reasons, will result in a failed road test.

f) I will not be involved in the scoring or result of the road test.

g) I understand that I am not permitted to enter an SGI facility during the testing appointment.

b) In the past 14 days, I have not returned from travel from any other location outside of Canada.

c)  In the past 14 days, I have not had close contact with somebody who is confirmed as having COVID-19.
