EXCERPTS FROM THE MINUTES OF THE SPECIAL MEETING OF THE BOARD

OF TRUSTEES OF NOTRE DAME OF SALAMAN COLLEGE, INC. HELD ON
FEBRUARY 21, 2020 AT NTDC, TACURONG CITY.

WHEREAS, the NOTRE DAME OF SALAMAN COLLEGE, INC, have been

recognized by Commission on Higher Education (CHED) eligible to implement Universal
Access for Quality Tertiary Education Act of 2017;

WHEREAS, in reference to implementing rules and regulations of Universal Access for

Quality Tertiary Education Act of 2017, there are amounts necessary to carry out provisions of
the Free Education;

WHEREAS, FR. REX C. BACERO, DCC the school president and/or JASPER B.
MIRAL the finance officer shall be the authorize signatories for the purpose of opening deposit
account in relation to the granting of Tertiary Education Subsidy (TES) funding;

NOW, THEREFORE, be it resolved as it is hereby resolved that FR. REX C.

BACERO, DCC the school president and/or JASPER B. MIRAL the finance officer, be
authorized, empowered and directed as they are hereby authorized, empowered and directed to
open separate deposit account exclusive for Tertiary Education Subsidy (TES) funding in the
account name of “NOTRE DAME OF SALAMAN COLLEGE – TES” with the Land Bank of
the Philippines, Tacurong, Sultan Kudarat.

RESOLVED, FURTHER, that FR. REX C. BACERO, DCC the school president and/or
JASPER B. MIRAL the finance officer, be authorized to execute, sign and deliver the necessary
documents needed in connection therewith.

Done this 21st day of February 2020 in the city of Tacurong, Province of Sultan
Kudarat, Philippines.
