How do you part-take or ensure your safety during the COVID-19 pandemic?
A. Introduction
To further our note-taking, we would like to record our conversations today. For your
information, only researchers on the project will be privy to the recorded audio tapes which
will be eventually destroyed after they are transcribed. In addition, you must sign a form
devised to meet our human subject requirements. Essentially, this document states that: (1) all
information will be held confidential, (2) your participation is voluntary and you may stop at
any time if you feel uncomfortable, and (3) we do not intend to inflict any harm. Thank you
for your agreeing to participate.
We have planned this interview to last no longer than two hours. During this time, we have
several questions that we would like to cover. If time begins to run short, it may be necessary
to interrupt you in order to push ahead and complete this line of questioning.
B. Background/Opening Questions

D. Closing Instructions
This will be the end of our interview. We would like to give a huge thanks for
participating and cooperating all throughout the interview. This recorded conversation that
consists our conversation will be a huge help. We will be using this as part of our gathered
data that would complete our said research study. As said before, the recorded audio tapes
will be destroyed once we completed the transcriptions. If the researcher/interviewee have
further questions that are related to the topic, we hope that your honored participation and
cooperation will be expected, but only if you agree with it. Thank You!
