Department will hold its Annual IT Week 2019 which aims foster innovation and collaboration
amongst IT faculty and students. One of your activities is to have a Mass Dance presentation
which will be participated in by the IT students and one of which is my son named Jessiemax
Escañan Rubio (1st year IT College Student).

In connection to this, the undersigned would like to ask a favor from your good office to allow
my son to be exempted for the Mass Dance due to some medical reasons. Since, the vital role of
the school is to maintain the safety and well being of the students, I would like to inform you that
my son has low level stamina which means he cannot participate an activity which demands a lot
of energy and sustainable physical or mental activity.
