EARIST Manila

Dear Ms. Ramos,

I enrolled in the B.S. in Electronics Engineering program for the 1st semester. Due to certain
circumstances, I have decided to return to my previous school in our province and complete
my degree.

Therefore, I am requesting the issuance of my credentials, specifically the Transcript of

Records (T.O.R.), Certificate of Good Moral, and Honorable Dismissal. I would greatly
appreciate the immediate issuance of these credentials. Kindly contact me as soon as they
become available so that I can submit them early to my new school.

Thank you for your cooperation.

Respectfully yours,

Clint Vincent E. Bais

Mrs. Loraine Santos

CIT Dean President
EARIST Manila

Dear Ms. Santos,

I am reaching out to express my dissatisfaction regarding the 10th-grade English instructor. I

want to clarify that I hold great respect for all of my teachers, and my intent in writing this
letter is rooted in my concern for our education.

The teacher, Ms. Heather, exhibits significant inconsistency in attending our class. On
numerous occasions, she has failed to arrive punctually, and, furthermore, she is absent from
our class at least three times every week.

In instances when she does manage to arrive, it is typically midway through the class, leaving
us with only 30 minutes of instruction time. This inconsistency is worrisome as it impacts the
quality of our English education and, subsequently, our performance in future English exams.

Therefore, I kindly request your intervention to ensure that Ms. Heather dedicates the
requisite effort and commitment needed to effectively teach us. Your attention to this matter
is greatly appreciated.

Sincerely,

Clint Vincent E. Bais
