Do not shampoo for two days after receiving your hair extensions. Always
wash your hair with your head back and not tilted forward. Never dry with a Street Address:
scrubbing action, instead wrap with a towel to absorb water.
Always dry bonds to prevent breakdown.

Always use anti-tap after every wet treatment on the bonds. Mix 1 line of Primary Phone:
anti-tap to 8 oz. of water in a spray bottle. If you cannot shampoo immediately
any time when the bonds are wet, spray anti-tap on the bonds and then
carefully dry. Shampoo, apply anti-tap and then dry the bonds as soon as Mobile:
possible. Do not use sulfur products or water that contains sulfur.

Brush your extensions three times a day, with recommended brush. Brush
row by row, scalp to ends, starting from the nape up. ALWAYS pull extensions
into a loose hair elastic while sleeping or any activity that has your hair blowing
in the wind. This is extremely important to prevent tangling or matting.

Return to SALON by milk + honey in two weeks for a checkup.

You may use curling irons, flat irons and hot rollers, but they MUST be kept at
least 1.5 inches from the hair extension bond.

Activities taking place in a constant damp environment such as aerobics, hot
yoga, steam baths, or saunas may lessen the longevity of your hair extensions.
Precautions should be taken to avoid these conditions.

Seawater and pools can cause the bonds to breakdown due to the constant
damp environment. To minimize these effects, wet hair completely in a shower
and apply anti-tap prior to swimming. After swimming, shampoo, apply
anti-tap and blow dry the bonds to prevent bond breakdown.

In consideration for receiving services at SALON by milk +
honey, I hereby release, waive, discharge, and covenant not
to sue Spa Partisan, Inc., its officers, agents, servants, and
employees or other business entities owned, operated or
controlled either in whole or in part by Spa Partisan Inc.
from any and all liability, claims, demands, actions, and
causes of action related to any loss, damage, or injury that
may be sustained by me or property belonging to me,
whether caused by negligence or otherwise, while
participating in such activity or while on the milk + honey
premises. I am fully aware of the risks involved and hazards
connected with spa and salon services, and I voluntarily
assume full responsibility for any risks of loss, property
damage, or personal injury, that may be sustained by me, or
any loss or damage to property owned by me as a result of
being engaged in such an activity, whether caused by the
negligence or otherwise.

Some hair and hair extension loss is normal and to be expected. Average loss is Date:
50-150 hairs a day. A full head application covers approximately one third of
the head. You can expect to see a small quantity of naturally released hairs
trapped in the attachments after some time. This is normal and should not be
interpreted as hair pulled out of the scalp by the extensions themselves. Daily Stylist Signature:
