CEMETERY RENTAL AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

HOLY CROSS PARISH CHURCH, Ampayon, Butuan City by REV. FR. JOIE L.
ACEDERA, Parish Priest, hereinafter called the FIRST PARTY; and

Emelita S. Ochea, legal age, Filipino, a resident of P- 1B Antongalon, Butuan City,

hereinafter called the SECOND PARTY.

WITNESSETH:

WHEREAS, the Holy Cross Parish, Ampayon, Butuan City, is the owner of a parcel of land
located at Antongalon, Butuan City, covered by Transfer Certificate Title No. RT-44985 which is
being used as a Catholic Cemetery;

WHEREAS, the SECOND PARTY requests the FIRST PARTY to lease a portion of the said
Catholic Cemetery consisting 4 ft. in width and 8ft in length for the burial of his/her relative
+ Thomas A. Ochea

NOW, THEREFORE, for and in consideration of the foregoing premises, the parties hereby agree
to the following terms and condition:

1. Amount for lease.- The lease shall be in the amount of THREE HUNDRED
PESOS(PHP 300.00) for every Five (5) years;

2. Period of the lease.- This Lease Agreement shall be for a period of Five (5) years,
renewable for another Five (5) years upon payment of the lease amount;

3. Non-renewal.- In case the SECOND PARTY or his/her representative or relative fails or

refuses to renew the lease agreement within one (1) year from the expiration of the
previous lease agreement, the FIRST PARTY shall have the option to transfer the
remains of the deceased to a suitable portion of the cemetery;

4. Administrative and supervision. - The Catholic Cemetery shall be under the direction,
administration and supervision of the Parish Priest or the Cemetery Committee. The
SECOND PARTY hereby agree to voluntarily abide by the rules and regulations which
maybe promulgated by the Parish Priest or the Cemetery Committee.

IN WITNESS WHEREOF, the parties have hereunto affixed their signatures this 16 th
day September 2018 at Butuan City.

Emelita S. Ochea, with their respective CTC/ID, known to me to be the same persons who
executed the foregoing instrument and they acknowledged to me that the same is their own free
and voluntary act and deed.
